In simplest terms a new vent pipe boot flashing is slid over the top of the existing one and the top edge of the pan is lapped beneath the overlying shingle course.
Now press the aluminum flashing flush to the shingle surface.
Be sure to clean the outside.

Pull the old flashing up over the vent pipe and scrape any old caulk and debris off the roof.
Before the flashing is pressed down to the surface of the roof you want to apply a bead of silicon on the shingles about an inch in from the edges.
Bend the corner flashing tightly around the corner.
Apply the vent pipe flashing according to instructions.
If you simply purchase the right materials you ll have to look for instructions online.
Rubber vent flashing automatically adjusts to any pitch.
If you do not wish to form your own corner flashing you can buy pre bent pieces and cut them to size or use a piece of corrugated aluminum which is easier to bend.
